Surrender declaration

I confirm that the information provided is accurate to the best of my knowledge. I confirm that I am the legal owner of this animal, or that I have lawful authority to surrender it. I have disclosed all known health and behavioural concerns.

Once the animal has been physically accepted and this form has been signed by both parties, I voluntarily transfer ownership to RGP Pets Dubbo. I understand that the surrender is final and that RGP Pets Dubbo may provide veterinary treatment, desex, vaccinate, rehabilitate, foster, rehome or transfer the animal to another suitable rescue. Decisions will be made in the animal’s best interests, including humane euthanasia if a veterinarian advises that it is necessary to prevent suffering.
